Treatment Guarantee

The following terms outline Discovery Point Retreat’s Treatment Guarantee. This guarantee is available to eligible clients who meet the criteria described below and is subject to the terms and conditions outlined herein. Discovery Point Retreat, operating under Discovery MSO, reserves the right to modify or discontinue this guarantee at its discretion.

Eligibility Requirements

In the event that a client experiences a relapse during the first nine (9) months after successful completion of Discovery Point Retreat’s full continuum of care treatment program, and the client completed a minimum of sixty (60) days of treatment, Discovery Point Retreat may cover the cost of attending a sixty (60) day Relapse Response Treatment Program one additional time.

For the purpose of this guarantee, a sixty (60) day treatment stay means sixty individual days completed in a level of care where services were delivered, received, and reimbursed by insurance.

To qualify for readmission under the Treatment Guarantee, the following conditions must be met:

• The client must have completed residential treatment (detox and/or residential) and outpatient treatment (PHP and IOP) through Discovery Point Retreat.
• The client must have completed detox, residential, PHP, and IOP levels of care without insurance denials or coverage lapses.
• The client must have paid all financial obligations associated with the first treatment program and have no outstanding balances owed to Discovery Point Retreat or any affiliated entities under Discovery MSO.
• The relapse must occur within nine (9) months of the client’s final discharge date from the IOP level of care.
• The client must have complied with the aftercare plan and discharge recommendations provided by the Discovery Point Retreat clinical team.
• The client must contact the Admissions Department seeking readmission within nine (9) months of the final discharge date from IOP.

Limitations

The following limitations apply to this offer:

• Travel expenses, third-party physician fees, and prescription medications are not included and remain the responsibility of the client.
• Services provided by outside medical providers or facilities (including emergency room visits, hospitalizations, urgent care visits, outside lab services, or prescriptions) are not included.
• Transportation to and from any Discovery Point Retreat facility is the responsibility of the client.
• The Treatment Guarantee is non-transferable and may only be used once.
• The complimentary sixty (60) day treatment period must be used consecutively within nine months of discharge.
• Any unauthorized departure, administrative discharge, or discharge Against Medical Advice (AMA) before completion of the program will result in forfeiture of the remaining treatment days under the guarantee.
• Discovery Point Retreat reserves the right to determine the most clinically appropriate facility placement. If no beds are available at the time of relapse, admission may be scheduled when space becomes available within a Discovery MSO-owned facility.
• The qualifying stay for the guarantee must be a paid stay.

Discovery Point Retreat reserves the right to deny participation in the guarantee program for individuals previously discharged for disruptive behavior, harm to other clients, or failure to comply with residential program policies.

Discovery Point Retreat also reserves the right to terminate a second treatment stay if any of the following occur:

• Disruptive behavior
• Harm to other clients
• Noncompliance with program rules or residential procedures
• Determination by medical or clinical providers that a higher level of care is required
• Determination by the facility administrator that continued treatment would not be therapeutically beneficial or would negatively affect the treatment environment

Administrative decisions related to these determinations are final.

Definitions

Relapse refers to a return to ongoing substance use or addictive behaviors rather than a single isolated event from which the individual quickly recovered.

Successful completion of treatment means completing all recommended levels of care in accordance with the individual treatment plan.

SMS Messaging Terms

Discovery MSO may offer SMS text messaging as a communication method for individuals who request information about services, admissions, scheduling, or program updates.

By providing your mobile phone number through website forms, admissions communications, or other contact methods, you consent to receive SMS text messages from Discovery MSO or Discovery Point Retreat.

These messages may include:

• Admissions communication
• Appointment reminders
• Program updates
• Follow-up communication related to treatment inquiries
• Support communications related to recovery services

Message Frequency

Message frequency may vary depending on your interaction with our admissions or clinical teams.

Opt-Out

You may opt out of SMS communications at any time by replying STOP to any message. After you send the message STOP, you will receive a confirmation message and will no longer receive SMS messages from us.

Message and Data Rates

Message and data rates may apply based on your mobile carrier and service plan.

Carrier Liability

Wireless carriers are not liable for delayed or undelivered messages.

Privacy and SMS Data Use

Discovery MSO respects your privacy and protects your personal information. Phone numbers and personal data collected for SMS communications will only be used to provide the services requested or related communications.

Mobile information will not be sold, rented, or shared with third parties for marketing purposes without your consent.

Information collected through SMS communications is handled in accordance with our Privacy Policy and applicable healthcare privacy laws.

Consent to Receive Communications

By submitting a form, contacting our admissions team, or providing your phone number through our website, you agree to receive communications from Discovery MSO, including phone calls, emails, and SMS messages related to treatment services and inquiries.

You may withdraw consent at any time by replying STOP to SMS messages or contacting our team directly.
